Jeffrey Croft recently wrote about the trials of one of his previous congregations whose pastor has been removed. I don't know about the circumstances, but it's a hard experience for all parties involved when a pastor is forced to step down, especially for reasons of personal morality.
How should the church, locally and connectionally, perform the removal of a pastor from office?

This is not very specific, but the process needs to be framed as a lament, in my opinion. Yes, it is a "hard experience for all," and everyone involved will be feeling much grief. We need to weep together about it.
